the light comes from the switch on the t/case. You're hubs might need to be serviced or there might be too much grease not letting them lock.

Have you by chance looked at the charcoal canister under the bed above spare tire? Check the hoses, connector and condition of the canister.

I did a search as well, some sort of similar but nothing exact. IMO, it has to be a fab shop because there are tooooo many cuts for a one off bumper.

If its a 97 as stated in your info, you have OBD II, you can have it read for free at any auto parts store. Just because the CEL is not on doesn't mean there isn't a code stored.
